Please supply the following information:
- Firewall Rule Number
- Source IP(s), MAC address (hardware address), DNS name(s), or Network(s)
- Destination IP(s), MAC address (hardware address), DNS name(s), or Network(s)
- Serivce and/or Ports (for example "web", "ssh", "ms-SQL", "tcp 51344", etc)
